    Comparative analysis of three different serum-free light chain assays in the diagnosis of multiple myeloma – Research



    Background:

    The lack of analysis methods and standardization are the core problems of serum free light chain (sFLC) detection in Multiple myeloma (MM). This study validated a new KHB sFLC assay through comparative analysis with conventional assays.


    Materials and methods:

    Serum samples from 97 hospitalized MM patients were continuously collected. KHB, Freelite and N Latex assays were used to detect sFLC. The Bland-Altman and Passing-Bablok regressions were used for methodological comparison and bias evaluation. Spearman’s test and Cohen’s kappa coefficients were used to evaluate the correlation and clinical concordance.


    Results:

    The sFLC results for KHB, Freelite, and N Latex showed a significant correlation. Passing-Bablok regression analysis revealed strong concordance between the KHB and N Latex for κFLC, and between KHB and Freelite assays for λFLC and FLC-ratio (κ/λ). When using N Latex and Freelite assays for sFLC determination, selecting iFLC/niFLC ≥ 20 or iFLC/niFLC ≥ 100 could lead to different clinical treatment decisions for approximately 9%∼12% of patients. When using KHB and Freelite assays for sFLC determination, selecting iFLC/niFLC ≥ 20 or iFLC/niFLC ≥ 100 could lead to different clinical treatment decisions for approximately 5%∼7% of patients.


    Conclusion:

    KHB, as a sFLC detection method based on polyclonal antibodies and immunoturbidimetric principles, has a good correlation between its detection results and freelite and N Latex. The absolute difference in sFLC results among the three assays increased with increasing sFLC concentration, and selecting the same cutoff value for iFLC/niFLC may lead to inconsistent clinical treatment decisions in some patients.

    [Immunological monitoring in systemic lupus erythematosus: Which markers for which lupus?] – Research


    To evaluate the usefulness of C3 and C4 fractions, CH50, sC5b-9, and anti-dsDNA and anti-C1q antibodies in the immunological monitoring of systemic lupus erythematosus (SLE), with or without renal involvement. This cross-sectional study included 74 SLE patients, 32 with renal involvement and 42 without. Immunological parameters were correlated with overall disease activity and with lupus nephritis (LN) activity. Significant differences were observed between the two groups. Renal involvement was strongly associated with anti-dsDNA (P = 0.001), anti-C1q (P = 0.001), low C3 levels (P < 0.0001), and elevated plasma levels of sC5b-9 (P = 0.002). In non-renal SLE, no correlation was found between disease activity and C3, C4, CH50, sC5b-9, anti-C1q, only anti-dsDNA correlated with disease activity (P = 0.02). In contrast, in renal SLE, LN activity correlated positively with anti-dsDNA (P = 0.012) and anti-C1q (P < 0.0001), and negatively with C3 (P = 0.008) and CH50 (P = 0.003). Elevated sC5b-9 levels were also significantly associated with active LN (P = 0.018). Receiver operating characteristic (ROC) curve analysis identified anti-C1q as the best marker of renal involvement, with an area under the curve of 0.929 and a negative predictive value (NPV) of 92% for and active LN. Immunological monitoring of SLE should be tailored to the clinical phenotype. Complement exploration is of limited value in SLE without renal involvement due to moderate activation during active disease. In renal SLE, complement markers prove particularly useful, with anti-C1q emerging as the best marker of lupus nephritis activity.

    Background:

    BRCA pathogenic sequence variants(PSVs)are believed to enhance local immune responses through the generation of tumor neoantigens, leading to increased tumor-infiltrating lymphocytes(TILs).


    Methods:

    In this study, we evaluated the relationship between BRCA-PSVs and both systemic and local tumor immune responses in 93 breast cancer patients who underwent BRCA genetic testing.


    Results:

    Among them, 16 patients(17.2%)were found to harbor BRCA- PSVs. Although no significant correlations were observed between BRCA-PSVs and absolute lymphocyte count(ALC), TILs, or CD8+ T-cell infiltration, a significant association was noted with the neutrophil-to-lymphocyte ratio(NLR)(p=0.006). Prognostic analysis showed that patients with low-TILs or high-CD8+ T-cell infiltration had significantly better overall survival( p=0.026 and p=0.020, respectively).


    Conclusions:

    These results suggest that local immune markers, particularly CD8+ T-cells, may serve as important prognostic factors in breast cancers harboring BRCA-PSVs. Further immunophenotypic and spatial profiling is warranted.

    Proteotranscriptomic Dissection of Breast Cancer T Cell States Identifies CD103+ Tfh-derived Cytotoxic Cells Linked to Immunotherapy Response – Research


    While cancer immunotherapies have primarily focused on activation of cytotoxic CD8 cells, CD4 T cell activity is also associated with survival and immunotherapeutic response in numerous cancers. We applied integrated single-cell RNA sequencing and multiplexed protein epitope profiling to breast cancer samples to resolve the complexity of immune cell states within the tumor microenvironment. This approach enhanced phenotypic resolution, identifying three distinct states within the CD4 T follicular helper-like (Tfh) cell cluster. A CXCR4high progenitor state gave rise to two differentiated states: an IGFL2high subset resembling conventional Tfh cells and localised to B cell-rich lymphoid aggregates, and a CD103+ subset, exhibiting features of tissue residency, exhaustion, and cytotoxicity, which co-localised with tumor foci. CD103+ Tfh-like cells were found to interact with CXCL10+ macrophages through production of CCL chemokines and CSF1. A higher CD103+ Tfh to IGFL2high Tfh ratio, together with the selective clonal expansion of the CD103+ subset, was strongly associated with improved tumour immunity and superior responses to anti-PD-1 checkpoint blockade, surpassing the predictive value of exhausted CD8 T cells. These findings integrate Tfh and CD4 with cytotoxic potential in breast cancer, offering new insight into anti-tumor immunity and response to checkpoint blockade.
